Restarting a GLP-1 After a Break: Why the Instructions Matter

After a supply gap or missed doses, restart instructions differ by medicine and time off treatment. Learn why checking with a pharmacist or prescriber matters.

A break in GLP-1 treatment can happen for lots of reasons: a missed refill, a supply problem, travel, side effects, a procedure or a change in coverage. "Can I just pick up where I left off?" sounds like a simple question, but the answer depends on the exact product, how long you were off it, your previous dose, other medicines and why the break happened.

Why your previous dose may not be the right restart point

Many GLP-1 medicines are started at a low dose and stepped up gradually, partly to make digestive side effects more manageable (more on why side effects can flare during dose escalation). After a gap, your prescriber may need to factor in how much time has passed and the product's own instructions.

The labels really do differ. Here's what the current US prescribing information says, as of October 2026:

  • Wegovy injection: for a single missed dose, take it if the next dose is more than 2 days away; if it's less than 2 days away, skip it. If 2 or more doses in a row are missed, the label says to restart dose escalation at a lower dose to reduce the risk of digestive side effects.
  • Wegovy tablets: if a daily dose is missed, skip it and take the next one the following day.
  • Zepbound: a missed dose can be taken within 4 days; after that, skip it and continue on the usual day.
  • Ozempic: a missed dose can be taken within 5 days; after that, skip it and continue on the usual day.

These rules cover specific situations and products, and other countries' labels may differ. A longer break is a conversation for your prescriber, not something a forum thread can settle. There's more detail in why missed-dose instructions differ by medicine.

What not to do

Don't double a dose to make up for missed treatment, inject extra, shorten the time between doses or follow an influencer's restart schedule. Don't use leftover, unverified or compounded medicine as a workaround. The FDA has reported hospitalizations linked to dosing errors and doses above the label, and warns about counterfeit and illegally sold GLP-1 products.

What information to provide

Tell your care team the exact brand and form (pen, vial or tablet), when you took your last dose, your prescribed dose, how long the break lasted, why it happened and whether you had side effects beforehand. Mention insulin or other diabetes medicines, any chance of pregnancy, planned surgery or procedures, and other health changes. If the break happened because you couldn't tolerate the medicine, that deserves a review before resuming.

Access interruptions are a care problem

If cost, coverage, supply or appointment access caused the gap, say so plainly. Your care team may be able to suggest legitimate options or plan closer monitoring. After restarting, get medical advice promptly for severe or persistent stomach pain, vomiting that stops you keeping fluids down, or signs of dehydration, rather than trying a self-directed reset.

The safest restart plan is specific to the medicine and to you. Checking first isn't overcautious; it takes the guesswork out of prescription treatment.
